Life-Threatening Health Conditions

If your student has a life-threatening health condition, a medication and/or treatment order and an Individual Health Plan must be in place before starting school. Contact your school nurse or your school’s main office as soon as possible.

Medication and Treatment Forms

In order for children of any age to receive medication at school, we must have a valid medication authorization form signed by a licensed health care provider and a parent/guardian. This is true for prescription, over-the-counter, and self-carry medications. The only medications a student may self-carry are inhalers, epinephrine autoinjectors, and diabetes treatment supplies. All other medication should be stored at school. Medication stored at school must be brought in by an adult and be in the original container showing the student’s name and dosage. Submit the authorization form prior to or along with the medication. Medication authorization forms are only good for the current school year.

If your student requires a medical treatment while at school, submit an authorization form signed by your healthcare provider and contact the school nurse.  The nurse will ensure a health care plan is in place to meet your student’s needs.  Treatment orders must be renewed annually.  If your student needs a treatment order not listed below, call your school nurse for assistance.

Special Dietary Accommodations

If your student has a special dietary need, submit the Special Dietary Accommodation Form to the Child Nutrition Department. This form, which requires the signature of a license healthcare professional, allows our Child Nutrition Department to provide alternate food choices for student’s in need. It is intended for health conditions such as allergies and digestive disabilities that have the potential to substantially impact a major life activity. It does not include personal diet preferences. The form remains in effect until canceled.

Diabetes

Type 1 diabetes is considered a life-threatening health condition and requires a treatment order and an Individual Health Plan be in place before your child can start school. Your provider will generate the treatment order. Your school nurse will use those orders to create an Individual Health Plan tailored to meet your child’s specific needs in the school setting. Newly diagnosed students are often encouraged by their provider to return to school as soon as possible. We do our best to support a quick return but we do require time to create the health plan and train staff, if necessary. Please coordinate a return to school date with your school nurse.

Each year, your child’s health care provider will update treatment orders for school. Please submit updated orders to your school’s main office as soon as possible.

Seizures/Epilepsy

If your child’s health care provider has generated a new or updated Seizure Action Plan for school, please provide a copy to the school nurse.

If your child has recently been diagnosed with seizures, contact your school nurse before returning to school. The school nurse will create an Individual Health Plan tailored to meet your child’s specific needs. We do our best to support a quick return to school but do require time to create the Individual Health Plan and train staff, if necessary. Please coordinate a return to school date with your school nurse.
